ALL students must submit the following prior to any rehearsal or performance starting July 8, 2024:
An updated UHS Band Health Form for the 2024-2025 school year.
A current WVSSAC Sports Physical Form (completed after May 1, 2024)
Be advised: Students who do not have a health form on file for summer band
will be sent home from rehearsal.
