ALL students must submit the following prior to any rehearsal or performance starting July 1, 2025:
An updated UHS Band Health Form for the 2025-2026 school year.
A current WVSSAC Sports Physical Form (completed after May 1, 2025)
Be advised: Students who do not have a UHS Band Health Form and their
WVSSAC Physical Form on file for summer band will be sent home from rehearsal.
